This proposal form DOES NOT offer you the ability to log in and out -- once started, you will need to COMPLETE IN FULL. To submit your proposal to present at a monthly meeting, we recommend the following steps:
- Print out the SAMPLE FORM and use it for planning the data you plan to submit.
- Complete the proposal form in its entirety.
- Move to the next data field by using your "tab" key.
Do not use bullets or numbering; plain text only, please
- Make sure all content is written into the proposal, DO NOT attach extra pages.
- Review for accuracy, completeness and neatness as the information provided will be printed in the chapter newsletter, postcard, emails and on the website.
- If submitting proposals on more than one topic or for more than one Speaking Opportunity, complete a separate proposal form for each topic.
