Print out the SAMPLE FORM and use it for planning the data you plan to submit.
Complete the proposal form in its entirety.
Move to the next data field by using your "tab" key.
Do not use bullets or numbering; plain text only, please
Make sure all content is written into the proposal, DO NOT attach extra pages.
Review for accuracy, completeness and neatness as the information provided will be printed in the chapter newsletter, postcard, emails and on the website.
If submitting proposals on more than one topic or for more than one Speaking Opportunity, complete a separate proposal form for each topic.
